so what we do instead is discourage people from going to the hospital for fear of being charged AND not being helped.

the difference is that since bob is now using medicaid, his costs are much less, and we pay much less overall. plus, there's no incentive to skip out on a bill, since there's no bill for him to pay.

Medicaid has nothing to do with the ACA, as if you qualify for Medicaid, you cannot enroll in Obamacare. So would you care to actually answer the question?
