You're not convincing me. I spent 7 years as a healthcare lobbyist, fighting insurance companies more than anyone else. But, as much as we hated insurance companies, we hated them because they were good at their jobs (namely, reducing payments to us). Medicare, on the other hand, might pay less per person, but it was almost guaranteed payments. Look at the sheet number of papers on Medicare FWA. It's obvious that it's a problem. Now, find a paper focused on private insurance FWA. I'm not sure that I've ever seen one.
