Neither physicians nor hospitals are required to accept Medicare, nor Medicaid, nor any private health insurance plan. I've already heard of many hospitals that are investigating what would happen to their bottom line if they simply refused to accept Medicare, and directly charged patients a fair price (cost plus 3%, for example).

What's going to happen if/when medical providers begin refusing to accept plans that don't pay even the cost of care?

There are no guidelines for the cost of anything. What is a fair price, and who decides? Its a fiasco, pretty much like gas. Thats just the cost of the policy of having no policy. If cost is basically not mandated, its not going to be affordable. Well that, and getting the fat cat middle man out asap.
