jesus christ. insurance is for catastrophe, it shouldn't be for care. people talk about it like it should be relevant to yearly checkups, colds, flu, minor broken bones, etc. it's not. it's for when you need open heart surgery, and should be treated as such. people have been abusing it which is why it is expensive. it's expensive because there's not enough doctors since everyone wants to "Get their money's worth" and because it's not being used for catastrophe but as standard operating procedure. you should be paying out of pocket if you want to go to the doctor about your cold you hypochondriacs

What would healthcare under John look like?

Why not just go straight single payer, and charge a small fee for each visit to the ER or the doctors office? This fee could then be used to offset the cost to the taxpayer. It should be small enough to not dissuade people from going altogether, but large enough to dissuade people from abusing the shit out of the system, which does happen (particularly as people age, and start fearing their inevitable demise, and so cost everyone else thousands upon thousands of dollars by coming in for every small ailment you can imagine).

Alternatively, if you insist on having two-tier, why would you make catastrophic care under insurance? That is precisely the kind of thing that needs to be covered under single payer. The second tier would be basic checkups and preventative medicine, which people would buy small policies for.
