The way to do it is set up a Department of Health and run it rather like the Department of Education. Doctors and nurses are public servants like teachers and hospitals are like schools. Anyone can go and it's free. If you don't want the inevitably mediocre health service the government provides, you can pay to go a private hospital, much like sending your kids to private school.
